Our recent custom training with the Ministry of Health and Medical Services in Fiji was a great success.
Our Improvement Academy (IA) faculty member, Associate Professor Peter Hibbert Program Manager, Australian Institute of Health Innovation, Faculty of Medicine Macquarie University, recently ran a custom Root Cause Analysis (RCA) and RCA Report Writing training session for the Ministry of Health and Medical Services in Fiji.
The Ministry of Health and Medical Services is responsible for the provision of medical services to the entire population of Fiji, which consists of approximately 920,000 permanent residents. In addition to this, similar services are provided to the expatriates visiting or living in the country.
Fiji’s geographical composition which consists of hundreds of dispersed islands, present unique challenges to the nation’s healthcare. More recently, the Fijian government announced plans for the refurbishment and upgrade to two major hospitals, and there are ongoing plans for improvement to the health sector’s ICT infrastructure and network.

The Ministry of Health and Medical Services have been doing important work to champion clinical governance and safety and quality perspectives into healthcare in Fiji.
Australia has a robust framework for clinical incident investigation, including root cause analysis methodology, which underpin globally recognised standards of care. During Associate Professor Hibbert’s visit to the Ministry of Health, he engaged staff with RCA and RCA report writing training, looking at steps to conduct an effective RCA for severe harm and frequent clinical incidents. The focus of the training was to introduce systems thinking and improving systems and processes to prevent harm. It also introduced effective report writing methods which facilitate effective communication and knowledge sharing within an organisation following a clinical incident.
